Case Manager - APC/DMHAS

Oaks Integrated Care is currently seeking a Case Manager to join the team at our adult Partial Care program in Mt. Holly.
The qualified candidate should have the ability to identify strengths & weaknesses and provide advocacy for consumer needs; have the ability to convey respect and gain trust with the population served; be computer literate with a strong knowledge of MS Word and Excel; and have excellent written and verbal skills.
Provide therapeutic group activities with a focus on wellness and recovery
Provide advocacy and case management services on behalf of consumers
Provide individual/family counseling to consumers and families
Provide liaison service from consumers to doctors
Maintain records to include progress notes, Individual recovery plans, discharge and transfer summaries and all other records required by agency policies
Be available to consumers and staff on a 24 hour basis during emergencies

Bachelor's Degree in Human Services or a related field
OR Associate's Degree plus two (2) years experience in providing human services
OR Relevant professional credential such as Certified Psychiatric Rehabilitation Practitioner, BA Degree in Social Work, Certified Rehabilitation Counselor, Certified Alcohol and Drug Counselor, BA Degree in Rehabilitation, or BA of Science in Nursing
Must possess a valid driver's license in good standing
Bilingual prefered.

Don't Be Fooled

The fraudster will send a check to the victim who has accepted a job. The check can be for multiple reasons such as signing bonus, supplies, etc. The victim will be instructed to deposit the check and use the money for any of these reasons and then instructed to send the remaining funds to the fraudster. The check will bounce and the victim is left responsible.
